If the dictation is going into OE Dept for HIM reports, maybe make the
drug and reaction queries that get answered during the transcription,
and then use those queries to notify PHA and or use a custom report that
can be manually printed or scheduled to print to PHA and someone in PHA
can document the ADR in PHA.
I think you can search the dept reports for a word or pharse (using L
(locate function in a custom report), but if you use a query for drug
and reaction, it would make this auditing/reporting to PHA much easier).
Also, you could add a query to the OE ADMN data screen for drug reaction
and based on the response it would notify PHA of the patient's drug
reaction one patient at a time(OA MSG sent by Yes response to Drug
Reaction).
You can create an order for the PHA Category called ADR or whatever and
have nursing enter the order when their patient has a reaction (ha
ha...I know....)
But, if a patient is having a reaction, the nurse is aware of this
(probably before the physician), so I am sure she/he is recording this
on paper?
If you can auto-mate that process, it can help you by notifying
different depts of the ADR, help create ADR stats reports, etc...
We have a Yes/No query on our End of Shift Summary, done twice a day and
it is required.

If the trigger query is Yes they had an ADR this shift, a report prints
to PHARMACY so they can follow-up on this issue.
Since we document the end of shift summary in NUR module, we attached
the custom ADR report to the intervention dictionary under activity
printing.

There is functionality in PHA to do ADR's. We have a CDS attached. Do
you have any archiving??
We built an MIS Medical Record Form and defined it to go the PCI. We
then had Meditech add this form in the PHA parameters to archive. Voila.
ADR's documented by pharmacists - go to PCI and to archive. If you don't
have archiving you could probably do something similar but print it out
and file.
Then to top it off I built an NPR based on the CDS which the pharmacists
can print on FAX to send off to any reporting bodies that need the
information.

Hello,
Our facility is Magic 5.4 SR3. We do not currently have NUR. We have
OE, PHA, PCI, ADM.
Currently when a doctor dictates at discharge that a patient experienced
an ADR for a specific drug, we have no automation to alert PHA of this
documentation.
We also do not have a system to then post this ADR to PCI or PHA module.
Any suggestions outside the NUR and CPOE Modules?
