Hi,

I know that this has been discussed several times before, but I have been
asked to find out how other hospitals have other disciplines, rather than
just nursing, document on the plan of care.

We are C/S 5.4 SR1, updating to C/S 5.5 SR2 next week.

Does Respiratory, Rehab, Dietary, Social Services, etc. add their
interventions directly to the worklist or do they add them onto the
appropriate problem of the care plan?  If they do NOT add to the plan of
care, how are you proving to JCAHO that it is an interdisciplinary plan of
care?  We are linking our notes to problems on the plan of care, but are
unsure if that will suffice.

Any advice would be welcome!
