For some reason, my replies to the entire L are bouncing back to me. Apologies if this message ends up there twice. This is for Kim and Dana.
We don't have 24 hour pharmacy services. Nurses enter only the meds they need to give. When signing off on orders, either at the time the orders are being transcribed or during a 24 hour chart check, if the med orders are not in EMAR, they are encouraged to write "Noted except EMAR meds". This way they aren't taking responsibility for something they shouldn't.
When I'm working as a staff nurse, the fact that the meds aren't in EMAR when I'm checking the orders is a pain, but I have to move on. I cannot stack my charts somewhere and check back into EMAR every few minutes to see if what I need is there. And I refuse to write "noted" for items - the transcription of which - I cannot yet check for accuracy. So I write, "noted except EMAR". Later in the shift, I try to check back and see if the meds are now there, and if they are, I'll pull the chart, conduct my comparison, and acknowledge the orders.
Nurses must not be writing "noted" at the bottom of a page of orders when the med orders are not yet in EMAR for them to check. They've got to have a way to indicate that they could not and did not check the orders transcribed by Pharmacy into the EMAR.
