This report is for an armband that is printed using HP LaserJet IV.
Iatric has a report on their website you should look at, however.  My
recollection is that it is a much simpler approach.

Does anyone have a sample NPR report that prints rotated barcodes
(vertical barcodes with horizontal text)?  Also, I know that
DAT=BAR.CODE produces a Code 39 bar code - can someone remind how one
gets the other symbology (128 I believe)?
