This form is used to inform everyone who needs to know that a new employee is on the way. It is routed via a distribution list to: Switchboard: Phone Book Update Plant Ops: Keys / Electronic Locks / Phone / Pager / Cell Phone Parking Tags IS: Network UN-PW / E-Mail & Internet Access / Meditech Access Printer Setup Mail Room: Mail Delivery Location Pharmacy: PYXIS Access This form is completed and routed the instant (just kidding) a manager initiates a Change of Status (hire/fire/transfer). This form has alleviated most of the problems related to having everything ready for a new employee on day 1. Like Gary, we have mangers who send the form the morning the new person arrives but these are rare. --- I've also attached our Acceptable Use Policy. The body of the policy addresses acceptable use of all technology. The Attachment is then used during new hire orientation. As the presenter covers each point, the new user initials the corresponding line indicating their understanding of that point. They AUP is then stored in the employee's HR file. However, we set up the logon information (usually) before they arrive for orientation, and give them that information at the time they sign the form. This leads to some inconsistency of timing; we have some managers who are very good about sending the request in time, others who don't get it until after the new employee has started. We use an Outlook form, so it's online, not paper. This has the advantage of being quick to fill out, and then everyone (HR, IS, other departments) are all notified simultaneously. In IS, we almost always accept the manager's request, and provide the requested access. In some cases, we may question a manager as to why a certain user needs certain access, or conversely, we may point out to the manager that the user needs certain access that's not requested on the form. But most of the time, we provide the access as requested. It works pretty well, the only issue being the managers who are perpetually late on their requests. Good luck!
