Gloria, We are using EDM with POM order entry for MEDS. It has been live about 1.5 years. We do not use POM in the inpatient environment yet. We are Magic ver 5.5 SR2. MD's enter about 95% of all med orders and have adapted very well. We use Pyxis and the ED machines are set to profiling. MD enters order, verified order created in PHA, order sent to ED Pyxis machines, Nurse accesses patient on machine and withdraws med from profile on machine, nurse documents med on eMAR. Note we have the parameter set to have orders go to PHA as verified because our pharmacy is not open 24/7 and waiting for PHA to verify, even when open, would delay care in the emergent situation. Because the ED has a need for many medications before orders are completed, the machines are set to allow override all the time for all meds. Now for the above to work, it is critical that a lot of care be given to building order strings that meet every need. You don't want MD's to pick a drug then modify the dose because as you identify it will link to the wrong product or no product in your dispensing machine. In our case, when the item is not available in the dispensing machine, a label prints in the pharmacy and the pharmacy will tube the item to the ED. After hours the nursing supervisor will go to another machine somewhere in the hospital to get a non stocked med if it is absolutely necessary. Because the label is in the pharmacy in the AM, the pharmacy can review the situation. Machine overrides do not go to the eMar. This is a parameter setting. Originally we did allow overrides to go to the E-MAR, but those withdrawals are not valid orders.. Ie no dose etc and we had duplicate entries on the eMar. So if a override is used, a EDM med order must be entered and the administration documented against the order. The med will be charged from the override, but the documentation will be stored in the order. At this time we are not using ADMIN billing in the ED. Not all meds are set to OE Y. But it doesn't matter anyway because we only build order strings for items that would be needed in the ED at this time. In addition, there is a location entry possible on the string entry to restrict the string to the ED locations to provide more control. I hope this info is helpful.
