The request is too general to build a report. You would need to:
* Define what specifically is a "lost charge"
* Specifically define what you want to return on the report in the way of 
fields (standard and calculated)
* Define what group of records you are wanting to look at.

So, pretty much like any other report. Break it down into selections and 
required data and go from there.

Has anyone come up with a good way to track lost charges?  Our Internal
Auditor has been charged with doing an audit on lost charges for both
inpatients and outpatients.  It seems like such a mammoth task, it's hard
to get started.
