The request is too general to build a report. You would need to: * Define what specifically is a "lost charge" * Specifically define what you want to return on the report in the way of fields (standard and calculated) * Define what group of records you are wanting to look at.
So, pretty much like any other report. Break it down into selections and required data and go from there.
