Here is a partial answer to your question:
NDC numbers are usually assigned in 10 digit form by the FDA. in a variety of
patterns: 4-4-2 or 5-3-2 (the number of numbers in the pattern of the NDC
number)
0049-3190-28 or 50458-325-28 there are a couple other patterns but never the
less Meditech uses 11 digits in the field for their NDC numbers and I have
seen
drugs with 11 digits on the boxes. the 11th digit is a 0 and if you are adding
a drug to the dictionary and just looking at the box (not pulling it from
the suspense file) you need to know if that 0 gets put at the beginning of the
first section of numbers or at the beginning of the second section of
numbers. That is why I have found that pulling drugs from the FSV suspense file
is preferable - even if I have the box in my hand. You get that 11th digit
in the right place.
To get the master NDC from all the drugs in your pharmacy dictionary, just
write a PHA.DRUG report and pull the fields for mnemonic, the trade name, the
generic, the strength and the NDC number and run them to an Excel file where
you can sort them however you want as you are working on them.
You need to make sure that the main NDC number in the drug is an active number
(Lots of us have old NDC numbers because as companies merge or new generic
are made or manufacturers quit making generics, new ndc numbers are created -
no one has time to keep up with this) This would be your first
thing to do. Have someone go through your wholesaler receipts and make sure the
NDC of what you bought is the NDC in the drug.
POSSIBLE EXCEPTION: this is what I heard an d I don't know if this is true or
not -
If you are going to do POE and your physicians will want to convert an
inpatient drug to an outpatient drug (one that you have in an RXM drug
dictionary)
the NDC of the inpatient drug must match the NDC of the outpatient drug. If
this is true and you use unit dose medications in house, you must put the
NDC for the unit dose product in you alt ndc field and the NDC for the BULK
bottle of the product in your main NDC - if that is what you have in RXM.
Truthfully - it may not matter.
Then for your ALT NDC field you go through your wholesaler list and first put
in the NDC's for every manufacturer for that med with that strength and form
in unit dose form and also put in the NDC number of the bulk form that you
would most likely by if the wholesaler cannot supply you with the unit dose
form.
If the med does not come as a unit dose form and the pharmacy will be putting
into unit dose dispensers from a bulk bottle, you need to put in the NDC
numbers of what they are packing from and alternates if the wholesaler cannot
supply that brand/manufacturer. Contracts change and different mfg's may be
on contract at different times which means adding NDC's to the ALT NDC field as
the contract changes. As companies merge or go out of business, NDC numbers
change.
Generally, the last 2 digits of the NDC number are package size numbers and
should not make a difference (still same company, same drug and strength)
but Meditech uses all the numbers and will not read just 8 or 9 numbers.
One drug can have several ndc numbers. For example A box of 6 Birth Control
Pill refill packs from one manufacturer may have the NDC of 12345-678-10.
Each compact refill (without the plastic compact) inside the box will have an
ndc of 12345-678-20. And each complete compact with the plastic outsides that
is individually packaged may have the ndc 12345-678-30.
So you need to know what you are scanning - the whole box, the refill or the
whole plastic thing with the refill. One week the pharmacy may have the
generic, the next week, they may have to substitute the brand name for 3 days
while the wholesaler waits for the generic to come in.
A box of 100 unit dose tablets may have one ndc, but each individual tablet may
have a different ndc printed on it.

Dear L:
We have had our first conference call with Meditech about BMV. We were talking
about NDC numbers, and they said that there was one “master” NDC number for a
drug that had 11 digits, and that all the manufacturer’s NDC numbers were only
10 digits. We then would need to update all of our NDC numbers to this master
one, and add all the manufacturer numbers to the ALT NDC number field.
Since I am relatively new to the field, I thought I understood that the NDC
number was different for each manufacturer, and that you didn’t need to change
all the NDC numbers in the drug dictionary – just use the one you use/buy most
often, and add the other numbers to the ALT NDC number field. I even have an
algorithm that someone sent from the L, about how to change a 10 digit number
to an 11 digit one.
Can someone shed some light on this for us? Do we need to get started with
updating our NDC numbers? Where do we find this master list???
