If I understand your question correctly, here's what we have:
 
We use FormsFast in our front-end registration areas.  To my knowledge, our registration areas have been satisfied with this vendor.
 
We also have a very large inventory of paper forms (either in use daily or used only for downtime) that will become part of the scanning process.  We are taking these old forms and scanning them into a desktop publishing software and adding the mnemonic barcode to it.

Add our site to your list of "Meditech scanning implementations".  We are working with our forms inventory now (what a job!!) and are beginning the naming conventions next week.  We went to dictionary training at Meditech last week and are scheduled to go live in March.

 

We are Meditech C/S 5.4.2.

 

I don't have answers for questions #1 and 2 below.  But to answer question #3 - we are naming ALL physician orders PO.

 

In regards to Carl's question for hardware, this is what we have ordered:

             Papa Bear scanners (Medical Records) - Fujitsu fi-5900C

                Baby Bear scanners - (Admissions) - Fujitsu ScanPartner fi-5120C color duplex

                No Mama Bear scanners for us.

We are going live early next year and would be interested in hearing what other sites are experiencing  as well.  We'd like to know: 

 

How are you naming conventions of forms? 

How detailed are your subcategories? 

Does every form(s - delete)  have a different mnemonic or are all physician orders  named  physician orders?

I need some help with hardware configuration with Meditech's Scanning Archiving.  I realize that there aren't many hospitals on this yet, but I don't have a list of the one's who are in implementation now.  If you have purchased this, and have already installed your hardware, would you mind sharing your configuration including backup design?  Sizes and brands of scanners would also be beneficial. We've developed our own terminology for these.  Papa Bear scanners, mama bear scanners, and baby bear scanners.  I really need to get all of this completed this week, so your response is appreciated.
