We are
looking for an efficient way to identify patients who meet the criteria for
core measures such as CHF, Community Acquired Pneumonia etc. We are not
currently doing concurrent coding so that is not an option. Any ideas how this
can be accomplished?
Also, who
is responsible to track and follow these patients to ensure that each criteria
is met?
