We use Access eForms as well and have removed most all multi-copy forms from our inventory.  There are a few forms however that must be multi-copy.  Those are printed by the print shop and kept in small quantities in locations where they are needed.  A bar-coded patient label is placed on those forms as they are used.  For those forms that didn't necessitate multi-part, but needed a copy, we put verbiage similar to this on the bottom of the form:
 
Original - Chart       Make copy for the patient
 
We moved from embossers to labels prior to the Access eForms implementation and were just printing them from Meditech.  We have found that even though we are able to generate forms with patient information on them, we still need labels for many of our patients (mostly) inpatients.  They are used for a wide variety of things, i.e. labeling specimen jars).

We are going to be using ACCESS EForms for our hospital forms soon.  First we are going up w/ Admissions, then on to other departments.  Currently we use NCR for several forms, one of which is for doctor's orders which are stamped w/ an embosser plate.  The Doc's write an order, we rip the copy underneath and send to Pharmacy.  We really would rather not have to deal w/ NCR paper in a copier, fearing they would get out of order, etc.  Our embosser machines are having end-of-life issues, so we'll probably need labels until the full project is up and running.  There are many other forms, but this is the hot-button form!

What are other facilities doing in regards to multi-page forms?
