We also use addendums for changes. I think once a report is signed, it is not supposed to be editted.

Subject: [MEDITECH-L] Pathology Report Process Issue

The following question is in reference to a Pathology Report that has already been electronically Signed Out.  What I am looking for is if any other Hospitals handle this differently.  Presently when a Path report has been completed at BRMC and electronically signed out the Pathologist is not allowed to go back into the report to make any changes.  If there was a mistake made in the reporting process instead of allowing him to go back into the Enter Corrcted Findings are to make the correction a separate Addendum report has to be created and attached with the corrections made on it.  The reasoning for this was that after the report was electronically signed out by the Pathologist it is available for review in the EMR.  If for any reason a physician would go into this report in the EMR, Review it, and base care on the diagnosis that was originally sent and then the report was changed and updated in the EMR without him knowing it could be a huge issue.  Are there other ways in which this is being handled and if so I would certainly like to hear from you.
