The most consistent way I have seen to charge by documentation is through Query Linking to an OE charge procedure. That way staff get a "suggested" charge, but can over ride it to something that makes more sense. And, I haven't see any of the issues that some have had when they attach a charge directly to an intervention.
Nearly all of our RT charges are generated via query links.
