It's very hard -- we went live last May. But it's worth sticking to your guns, I believe. If users ALWAYS have to change the time back to the assessment time (because it's never Document Now), it will take them longer to document AND you introduce typing errors. True, there are times when the nurse just needs both hands to take care of the patient. But if she has a hand available to write it down, you should STRONGLY encourage entering it straight into the computer instead. The quick entries -- recording an output for instance -- are burdened with the need for log-in time. But in ICU, the nurse can often keep a patient's screens open and with good screen design, not everything has to wait until later. The best selling point of the system is that users (with access) can see the data ASAP from any location. If documentation is done later, you are losing that benefit. Your best tool in critical units will be the Process Flowsheet. Our ICU and PACU users do frequent real time documentation from the Process Flowsheet and it works very well. It's not the most mature of products so don't hesitate to question your specialist; I know there are a number of DTSs out there, they created one for us just last week when we found an inconsistency and asked for resolution. In Labor & Delivery here, they don't do so well with the real time thing. The doctors wage a campaign for an underground paper system, they claim they can't find what they need in the computer. So the nurses don't have time to document in NUR because they are busy writing paper flowsheets. then at shift change, everyone is furiously typing away. Their manager is working on strategies to promote change in the unit. I wish you luck. Get in touch if you have any questions.
