Ann, you could consider using the Schedule CDS Query Checks on page 2 of
the Appointment Type (or on a CDS attached to the specific appointment
types). When scheduling a rehab appointment, you could set up queries
that asked the appropriate questions (Is this patient Medicare? Is
another MCR patient booked in this slot?) that would then allow you to
trigger a warning to the scheduler, or even reject the appointment.
That's not 100% automated, of course, but it would likely serve your
purpose. 

Same thing with the second question; on your CDS associated with the
appointment type, you could remind the scheduler of the Medicare
on-the-hour rule.
