This is follow-up on my original post (see below) to
the L-List.  Sorry I'm so late getting back to folks
on this ... we've had our hands full trying to
straighten all this out!!

Since my post on this subject to the L-List, we have
had to completely change how orders are entered ... as
in they are NOT!!   PHA was spending up to 6 hours
once they came in in the morning correcting nursing's
mistakes on order entry.

What we have gone to is to have nurses keep a copy of
the written medication order with them thru the night
shift (when PHA is not in house) and to use this as a
reminder of when new meds are due.

When a med is due, they use Quick Charge/Document to
Enter/Document that one time dose.  We have removed EO
and EM from their verb strips.

The wisdom of folks on the L-list is really showing,
since the vast majority of the posts received indicate
that nurses just cannot do order entry accurately. 
Our pharmacy realized it takes MONTHS to train a
pharmacy tech (who does nothing but enter orders all
the time) to do this accurately and effectively.  To
expect nursing, who does it only occasionally to do it
well with just one training course really is
unreasonable.

My recommendation to others about to embark on this is
to either utilize a virtual pharmacy order entry
(we'll probably eventually go this route) or to use
the method we're currently doing.
