We include Last Name, First Name, visit number, master account number, birth date, age, sex, date of service. and then barcode for the visit number off to the right. Depending on which version of Meditech you are, you may want to look at placement of your barcode and size. I have seen discussions on the L of placing the barcode vertically which would help nursing greatly when scanning the armband (less curvature).
