Maybe you could adapt this to CPOE:

We have a test in OE called AOT (Add-on Test).  OE users order it only for labs 
and only when they know we already have specimens.  There is a CDS which is 
free-text for what to add.  I didn't use a pointer because I wanted them to be 
able to add several tests at once if they needed to.  It interfaces to LAB and 
it prints in the lab on a printer that is monitored all the time.  

The lab checks the order and determines whether they can run the tests, given 
the types of specimens, collection and handling issues, etc.  If the can add 
the tests, they add them to the existing requisition.  Then they result the AOT 
as D for done.  

If they can't do the tests, the result it as ND for not done, and make a 
followup phone call to the nursing unit, documenting with internal comments.

We set this up to get rid of the hassle with the nursing unit needing to call, 
lab scrambling to find specs while the nursing unit is on the phone, 
documenting the read-backs, and all that wasted time. 

Now, nursing and lab don't have to get on the phone unless the order can't be 
completed.  There is a nice audit trail in OE and the corresponding reports in 
LAB to show the sequence of events.  Nursing and Lab users have been extremely 
satisfied with this system.

I've seen success with an order called "ADD ON", which has a free text 
query.  Then its in the lab's ballpark to decide if there needs to be a 
new draw.

Good Morning,

We are Magic 5.5 and are starting planning for CPOE next year.  I was 
wondering how people are handling add on lab orders with CPOE.  It is 
unrealistic to assume that a provider is going to be knowledgeable of the 
hundreds of different add on criteria to determine whether to order a test 
as a new draw or a test called add on that allows them to comment what 
test is requested for add on.  Please let me know if you have a creative 
solution to this, or how your lab is handling the combining of these 
orders.
