Carla, We do have all you seem to be looking for and we got it through Meditech. We print on a 3-part form I get from The Relizon Company. Try this number 405-949-9826. Our sales rep is Karrin Ferguson and a reference number may be 2250-2412970. If none of that helps let me know and I'll look somewhere else. The report we use is a custom we built that is attached on page 5 of the Customer Defined Parameters. I didn't build it and don't have access to that report.
