We ask "Pain in the last 48 hours" and go from there..

Hx of pain seems to me that it would tie your staff up hearing about pain
episodes from ages ago that might not be relevant  to this visit

 

I think that JCAHO is looking to make sure that the patient did not get
relief in the ER and then fell through the cracks on the units.

Regarding pain assessments, it is a good idea to look at this.  We just had
a jcaho visit and although we assessed for pain, we were asking were they in
pain? Y/N  and they wanted us to ask, "Is there any history of pain"  "What
helps alleviate the pain" "How well are the meds working?"  etc.  don't know
if all surveyors will want that but it is a start.  I think having a
different question on each form would make sense to keep the thought process
moving in the direction of assessing the whole realm of pain for the
patient.  (and maybe liven up the charting a bit! :-))
