We are C/S 5.5.2 and in the beginning stages of computerizing our manual
Admission Surveillance Cultures protocol. This protocol is initiated any time
a patient is admitted with any of the following 3 factors:
* hospitalized for two or more days in the preceding 90 days
* arrives from another healthcare facility such as a nursing home, a
critical access hospital, a SNF, or a long term/acute care facility
* the presence of a decubitus ulcer or a draining wound
When this protocol is initiated, there are specific cultures that are ordered
on the patient. We are thinking of creating an Admission Surveillance Cultures
assessment that will ask the above three questions. If any are answered yes,
then other questions will be asked to help determine what orders are needed.
Based on those answers, we are looking at using query links to order the
particular tests and will also create a clinical notification to alert the
appropriate areas.
Before we make a final decision on the how to approach this project, we were
wondering what other hospitals may be doing to address this issue?
We are not aware of a way to trigger any message, notification or order during
the admission process if the admission source is one that indicates the patient
is from another facility as described above.
