I agree.  It COULD be done if all of the needed info was available in
the system, and one lone overworked programmer had a few weeks to
dedicate time solely to the project.  I studied the specs and they are a
monster!  At our site, the MSO has opted for a "scaled down" version
that I created.  About every 18 months they will ask me if I can expand
on the foundation I've created, and my answer is always the same (yes,
but...).

Interesting question. Now pardon me, because I'm not thinking globally,
just  on an indicator-by indicated basis. But for example, you could
have an NPR that pulls all patients with admitting diagnosis of
pneumonia, then also pulls their time of admission, time of first
antibiotic, and time of ABG/SaO2, then automatically calculates whether
things were done within the time frame allowed, and displays compliance
statistics. I couldn't write such a report to save my soul, but I know
people who can. The hangup in my facility would be that we're not up
with BMV/eMAR in the ED, where most first doses are given, so that data
would not be available electronically.
 
Anyway, in theory, a lot of the Core Measure stuff could be assembled by
an NPR report. But you knew that already. I'm just sort of thinking "out
loud" and trying to keep the dialog going, because I think this is an
interesting and important subject.
