Our pathologists are interested in how others are handling anatomic
pathology reports:

 

As a first step in the process of instilling uniformity in our system of
issuing of Corrected Reports, etc, I should like to obtain consensus
from all of you that you are comfortable adhering strictly to the
following definitions, which have been endorsed in recent publications
by both the Association of Directors of Anatomic and Surgical Pathology
and the College of American Pathologists:

 

1.      Where the DIAGNOSIS CHANGES: Amended or revised report. 
2.      Changes in the report other than the diagnosis: Corrected
report. 
3.      ADDITIONAL information, no changes to the original report:
Addendum report.
