I have generally seen these questions asked as part of the nursing admission
assessment:
Recent foreign travel?
Where?
Returned when?
Along with immunization information this seems to make the most sense

Hello,
With all the Pandemic stuff going on around the world today, has anyone
been
asked by their Infection Control group on the ability to ask patients
questions like "have you traveled out of the country lately"? "And if so,
where"?
Also, for those who have been asked this, who is capturing this data?
Clinical departments or registration? We don't really want to start asking
every registration these questions.
We are a Magic 5.5 facility and was hoping someone else is getting hit with
this as well.
Any assistance would be greatly appreciated.
