We began the Form Fast roll out to all nursing units starting in
December 2005.  We began with the Discharge Instruction form which was a
3 part form and moved it to a 2 part NCR form.  Things worked for a few
months and then there were problems identified by the HIM department
because the nursing units were not making a photocopy and the records
were incomplete - the doctor took a copy, gave one to the patient and
the chart copy never surfaced..  We ended up reverting back to a 3 part
form for that one. 

 

With the exception of the Discharge Instruction form we have
successfully migrated about 85% of the forms into Form Fast, physician
orders, consults, progress notes, department specific forms, etc and all
print with out multi-part form and only NCR forms - we even had 2 part
Pink/Pink to support the physician consult sheets.

 

It is an adjustment to the staff because of the lack of glue, but once
they used it enough, and we did have administration's support there
weren't any complaints.  Just have good staplers!

 

One word of caution with the NCR paper - make sure the staff is fully
"trained" on how to load the paper.  If the paper is loaded incorrectly
it will not write through causing multiple issues.  I think this was the
biggest adjustment of all!!

We use Form Fast and have a committee that is going through all
departments and converting all of our forms, such as physicians orders
to electronic forms.  One of the issues we have run into is getting
departments to give up there multi-part forms.  We would like to convert
the multi-part forms to NCR paper, but we are getting a lot of
resistance.  Having to line up the NCR paper after it comes off of the
printer has been a complaint and the worry that the text written on the
first page of the NCR paper filters through to the right lines on the
2nd or 3rd page.  I would love to get some feedback from any facility
who has implemented Form Fast and what your process is for multi-part
forms and are you using NCR paper.  Give me the good, the bad and the
ugly.
