We had the same problem, a physician who worked part-time as a Hospitalist, but also maintained a private practice. Originally we created two Provider mnemonics and used one for private practice and one for his Hospitalist role. This has multiple problems, especially with nursing and admitting choosing the wrong provider mnemonic. Since then we've had several part-time Hospitalists and private practice physicians and we have one provider mnemonic.
We have created three new Hospital Services: ODOHOSP: Hospitalist Observation pts MEDHOSP: Hospitalist Medical pts SURGHOSP: Hospitalist Surgical pts and TCUHOSP: Hospitalist Transitional Care Unit pts. We then added a query to our Order Entry consult CDS that asks if this consult is a Hospitalist consult. We created a NPR report that looks at pt's by Hospital Service and also pulls in all those consults for Hospitalists. We contacted Meditech in regards to doing a PCI custom list that would search for pt's by Service, but they were unwilling to do this, so our physicians have to rely on the printed NPR report for their list of pts. We also use Valco's Dataglider Physician Portal and they are in the process of creating a role for Hospitalists that will pull in all pt's with one of the 4 Hospitalists services and all those consults for Hospitalist. This should give them an accurate inpt list. First, she is a credentialed staff physician with her own private practice. Second, she is a contracted physician for our ED through an outside company.
