I do not know this JCAHO standard, can someone tell me is this is
required for all meds, even meds the patient has been on for years, or
just a new med while in the hospital?

How do you know it is a new med, maybe they took it before, but is not
listed on their home meds list?

How do you accomplish documentation of Patient Education when the first
dose of a medication is administered in the inpatient & outpatient
setting?

Thanks,
