 We have been having quite a problem with coming up with a process to
capture patients that have already had their pneumonia vaccines
consistently.   We currently have a question in our admission history
that asks "Pneumococcal Vaccine Status" which has a look up with options
such as "Given prior to admission" or "None/Not Documented".    Whether
the nurses arent asking the question or are getting inaccurate responses
from the patient, we are finding cases where the patient had the vaccine
but it was documented incorrectly.   I have written NPR reports that
pull the query responses from that question and also one that looks in
the pharmacy module to see when the pneumonia vaccine has been
administered, but they want a "one stop shopping" place to see the
information.   I would like to hear what issues other facilities have
had with this query and how they are handling the problems.
