With our more acute patients we are having a problem with documenting I&Os and IV Intake on the IV Spreadsheet. We currently document I&O hourly on all of our patients. When documenting on the spreadsheet this means adding a column for every hour than documenting the intake and output on both the IV spreadsheet and the I&O. This is pretty cumbersome and as acuity soars, so do tempers. But we see this as the only way to get the Intake total per fluid per hour to appear on the I&O panel. Are we wrong? is there another way? How are other sites handeling this? Thanks Danielle J.
