HI - I have a question about the PRN reasons for pharmacy and I suppose it will 
trickle over to MAR, eMAR, etc.  Right now I have a nice PRN
reason dictionary built. However, we are currently not using it. We are putting 
the reasons into the label comments. This is why:
In order to select a prn reason from the prn reason dictionary, you have to put 
the schedule as PRN.
If you put the schedule as PRN, the order winds up on the PRN section of the 
MAR. Okay - so far so good. But not all orders with reasons are PRN.
You may have an order for  Ibuprofen 600 mg q4H for arthritis pain. Since this 
not a prn RX, it is a Scheduled RX, you have to enter the schedule as SCH
to get it to go to the right section on the printed MAR. I sort of thought I 
remembered somewhere reading that there might be a regulation coming in the
future
that would require "medical necessity (i.e. a reason) for meds. So I would like 
to be able to get to the PRN reason dictionary even if the order is not PRN
(at least when doing pharmacy orders). Does anyone else think this would be a 
good idea?
(My other idea a while back was to somehow be able to link ICD-9 codes to 
reasons but I realized that that would be kind of a bottomless pit)

Jeff,

The 5.6 EMAR is a complete rewrite and it may well show the PRN reason
more prominently.  Like I said before, we do require the PRN reason and
we have not had any problem with nursing missing the reason.  We do
print the PRN reason on the med label.

I would be concerned that the doc will not enter the reason as a
comment.

I planned to use PRN reasons with eMAR and even built the dictionary,
but now am hesitating. We are Magic 5.5 and when the nurses scroll
through the meds on the eMAR, the PRN reason seems very difficult to
spot. The label comments (where we have traditionally entered prn
reasons) do display quite prominently though so I guess we will stick
with this practice. Am I missing something - I would love to hear the
experiences of others regarding what works best for the nurses. We will
be soon be seeing 5.6 and I hope this will make the decision easier.
