We do our DVT risk assessments on all admissions over the age of 12.  It is a 
separate intervention.  Based on the total, then if the pt is at risk, the 
scale prints out in an OA msg to put on the chart for the physician.  Also, at 
the bottom of the assessment, the nurse selects if this pt is a Medical, 
Surgical (non-orthopedic), or Orthopedic patient.  Then based on that answer, a 
physician's order sheet prints out and is placed on the patient's chart.
Lastly, the DVT assessment is done q3days, just like our Fall Risk and our 
Pressure Ulcer.  The staff is already in tune to do those q3days, so this fits 
into that routine.

did you build this using CDS screens and Queries or some other means besides
Meditech?
How do you que to re-assess if there is a problem?
