Hello everyone. We are in the process of working on our med reconcilation process for our outpatients. What processes is everyone doing to collect an accurate patient home medication list for outpatients, including the ER , radiology , ambulatory surgery and your clinics if you have them ? How do you get the information ? Is it electronic format ? What do you do to change it if needed ? We are trying to come up with something that works for everyone here. Thanks in advance. Gale Y.
