We haven't done this, but I believe that the best thing to do would be to build a table with ICD-9 codes cross-referenced to formulary vendor codes (FDB in our case) and script them in as a disease state. However, one would have to differentiate chronic and acute disease states and script out the acute ones on discharge. Hi everyone, I was wondering if anyone has had luck with writting a report that can look at the patient's coded Disease(s) via Pharmacy and cross check their profile in PHA.RX to see if they are on...aren't on a particular medication (based on PHA.DRUG). I've tried to write it in PHA.RX but cannot seem to get it to work. Anybody have any luck with something similar? Thanks!
