We use the pre-printed red and white forms. I was able to get the alignment correct with the HP4250 printer using the 4100 PCL 6 driver. Electronic forms settings are as follows: CPI 10, Left Margin .2, LPI 6, Top Margin .252. I believe the default driver for the printer is actually the 4100 PCL 53e, which I could not align. The driver made a huge difference.

The right and left margins are good but it needs to come up 2 lines. We used an HR printer to test so didn't want to install a new driver. The HR printer has the post script driver installed.
