Barb,

 

We are undertaking a similar task in preparation for our SCA.  We are
developing our forms using NPR and have chosen the common sense route.  You
have up to 15 characters if I remember correctly.  For a form titled
'Congestive Heart Failure Pathways' we would use CHFPATH, 'Physician Orders'
is PHYORD, etc...
