We have been using the attached form for a couple of years. There are pop-ups
for three of the questions.
Also looks like I need to update the screen a little, but you'll get the idea.

We have been struggling with our IV documentation screen (where the nurse
documents Inserts, tubing changes, sites etc) it just seems to be confusing.
