I have seen it at 3 days. I remember a situation in which it emerged, however, that certain physician practitioners who were using the Patient Notes function (can you believe it? Bless their souls!) were documenting weeks after the patient was discharged. At the time, we didn't seem to have a way to stop this from happening, since the physicians had access to the accounts. So we had our consultant write a report that would autoprint to the department manager's desk if anyone wrote a note more than X days after discharge.
