We do not track which micor reports are completed after discharge. The provider 
gets a report on paper or electronically depending on the office when it is 
complete regardless if the are discharged/inpt/outpt and it is up to the 
provider to follow up on their patient. The only time we do call is if it is a 
critical result such as sepsis or our more nasty bugs but not for a UTI.

We are a critical access hospital with an average length of stay of 2.7 days.  
Leadership believes that there is a need for the hospital to follow up on 
microbiology reports that result after discharge to insure that treatment 
reflects the final microbiology report.  Does anyone have a process that tracks 
microbiology reports that go final after discharge?  What department is 
responsible for insuring that providers are aware of positive final 
microbiology reports that result after discharge?  What reports do you use to 
support this process?
