Dear List, With the coming of Med Reconciliation, these two topics take on a whole new meaning for Nursing Documentation. So I am looking for different ideas on how to document these?
Currently we have an Admission Nursing Assessment where these are documented, then pulled to PCI for viewing. But what about updates? What if meds or allergies need to be added after admission? Do you use a CDS, Admin Data Screen, Edit, or what? Are your med history screens and allergy screens separate or together on one screen?
