Dear List, As our site gets closer to bringing Clinical Review live, we are looking forward to building Inpatient Provider Documentation. Our first attempt to build a template was to bring in our current Physician Progress Note into Provider Documentation. Since we used the formatted data of vital signs, I & Os, medications and labs, the size of the "finished" document, with just formatted data, was about four pages when our current note is only one page. In looking at what might bring the document down from four pages, we asked Meditech if the font size could be altered and we were told it not able to be changed.
SO, I am trying to see that other sites have built in Provider Documentation. * What type of notes? * Individual Notes per Provider? * Hospital Mandate for a Provider to use Provider Documentation (i.e. Bad Handwriting) * Any impact on coding? * Success stories? * Using voice in Provider Documentation?
