Well, I have some good news (and hopefully my last post on the topic).

After many tedious and excruiating hours of testing, I think I have found the 
settings that will work.  The keys to getting this to work are:

1. First, finding the CPI settings that will allow ALL characters on the line 
to print.
2. Then, finding the proper horizontal edge-to-edge setting that aligns the 
form correctly.

By no means is the form perfect.  Unlike Meditech's programming for the UB92, 
which they did a very good job on and is almost flawless, Meditech's 
programming of the UB04 has left a lot to be desired; in fact, I would be 
embarrassed to send this form out to payers with the way that some of the 
fields are lined-up within the blocks.  But it DOES print all of the 
characters, and for the most part, the characters that don't fit nicely within 
the blocks are probably not critical data fields.

Here are the settings that I have found work the best:

ELF:
     CPI:  10.12   (this is the key; it is the smallest value that allows all 
characters on the line to print)
     Left Margin:    0.20
     LPI:   6
     Top Margin:  0.095

HP 4250 Printer Quality:
     X1:  -4
      Y:   0
