eMar 1st dose:

Yes, we've developed a way to do this with a custom Medi-tech program. It took
6 months to get this accomplished (we started June, 2006)- we (pharmacy)
always felt there was a way and finally got the right people involved & got it
done.

Here's how it works:

The first time a med is given, a CDS pop-up occurs when the nurse scans the bar
code of the med; this pop-up screen does the following 3 things...

1. rerminds the nurse of the 1st dose of a new med, need to educate the
patient/print the drug info sheet, and monitor for side effects
2. requires the nurse to document who was taught (patient, spouse, care-giver,
etc.)
3. requries the nurse to document level of understanding.

We instituted 3 weeks ago, and seems to be going well. It's not a big hassle
for the nurses as they can document right @ time of 1st dose. They don't have
to go somewhere else to document. The nurse documentation becomes a part of
the history of the medication order. The next time the med is given, the CDS
query does't re-occur, since the program searches for the generic name of the
med.

For example, the first dose of coumadin, the nurse gets the CDS screen. When
the doses changes (often daily), the query doesn't show again, since it's the
same drug (generic name), even though the dose keeps changing. Contact us
if you are interested. I have a fantastic/visionary staff pharmacist who
figures out most of these types of issues- I'm blessed.

Subject: [MEDITECH-L] eMAR 1st dose

Has anyone had any experience in the eMAR with electronic, 1st dose
documentation? I understand this is something JCAHO will be looking at
closely.
