Terri, this is fascinating. I love how quickly the nurse would be able to activate or deactivate a problem. I hope you don't mind some questions.
Are the fields for "R/T", "goals" and "action taken" all free text? What if you didn't have to have goals and priority numbers? Would that simply make things easier all the way around, or would it detract from patient care? How would you compare the kinds of things the nurses would document in the "action taken" fields with the content of your interventions screens?
