Hi Rip, Here is a copy or our Rad req. Probably more then you want or need but you can remove what you don't want. There are also 2 standard Meditech UDF req formats that you could initialize: RADRW.EXAM.rad.udf.req.format.1 and RADRW.EXAM.rad.udf.req.format.2. Questions, let me know.
