Susan Wozniak, Elmhurst Memorial Healthcare For the Medication Reconciliation Process and to comply with the JCAHO Patient Safety Goal, the next provider of care is to receive a list of the patients home meds or the meds that the patient was sent home with. What do you do in the case of Non-Staff Physicians? Do you send the list to Medical Records with the Non-Staff Physician information and they mail the list to them? Are some sites doing this electronically? Any information you can share would be greatly appreciated.
