Hi all, I am looking for an ORM report that contains Pt name, Pt U#, Date of operation, Start and end time of operation, Name of surgeon and any assistant, Name of nursing personnel (scrub & circulating), Type of anesthesia used & name of person administering it, Operation performed, Pre & post-op diagnosis, and Age of pt. The Daily Log in ORM has most of this but is missing a few things. The OR Director was told this is the minimum requirements for CMS. Thanks in advance. Jo
