Here is what we came up with for our Discharge portion. It pulls in the
current hospital meds, the home meds (from a NUR CDS) and then gives a
section for doc to write the new scripts and give to patient for their
pharmacy.  In our state, it is valid for controlled medications.  We are
MAGIC 5.6 PP4.

What are facilities using for Medication Reconciliation on Discharge and
transfer?  We have the admission to the hospital one working, and now
are concentrating on the within facility transfer or discharge
reconciliation.
