Here is what we came up with for our Discharge portion. It pulls in the current hospital meds, the home meds (from a NUR CDS) and then gives a section for doc to write the new scripts and give to patient for their pharmacy. In our state, it is valid for controlled medications. We are MAGIC 5.6 PP4.
