We want to get started with using RXM. One of the purposes is for Med Reconciliation here in our hospital.
Could you give us some direction on how to make sure things are set up correctly as far as actual use? Also the pharmacist is asking about Home Meds- Currently our nurses are listing the patients home meds in a pre assessment form that does not go anywhere except in NUR. What would be the best way for us to get that home med list at the beginning of the hospital admission? Just have the nurses use the Update Med List from the beginning? Your help would be appreciated. Thanks!
