Hello Everyone - Thanks to all who responded to this question. It was very helpful. For those who are interested, most agencies are asking employees to verify the inforamtion. Invoices from child care providers with Tax I.D or Social Security number is required. Copies of cancelled checks don't suffice. Apparently a copy of the bill for services is adequate. A key outstanding question, though, is whether the employer needs to get a SIGNATURE from the provider stating that the services were provided. A good source of information, according to Lisa DeAngelis at Brisbane is the the Government Finance Officer Association book titled "Flexible Benefits Plans". Thanks Lisa for the information.
