We've had a few conversations about tap in various forums, but haven't
really put anything concrete here so that people could get the whole
picture.

Comments welcome. We've got enough implementations to have ironed out
a lot of the issues (three or four people have done it it many
languages), but I'm sure there's plenty of room left for discussion.
The document is somewhat incomplete, but I figured publishing
something is better than waiting to get it perfect.
